Taking initial referral information and starting the referral process.
Registers new patients, assists nursing staff with coordination of patient needs with case management or referral sources.
Coordinating with other departments to initiate a patient's therapy.
Documents all aspects of referral into the EHR system.
Enters patient information into the computer through multiple systems Notifies staff of patient entry into the computer and preliminary reimbursement approval.
Notifies case management team of order problems that need to be resolved prior to start of service.
Requests patient documentation as needed to verify patient meets qualifications Excellent decision making skills.
Excellent communication skills.
Ability to work under time constraints and multitasking demands Capable of handling demanding situations in a professional and courteous manner.
